Seasons offers both short term rehabilitation and long-term skilled nursing care, all in an environment that feels like home.

SHORT TERM REHABILITATION

IN-HOUSE PHYSICAL THERAPY

Gait and stair assessment and training

Balance assessment and training with fall-risk management

Theraputic excercise programs to improve range of motion, strength and endurance

Pain management, ulatrasound, electrical stimulation and manual techniques

Home assessments to ensure a safe return home, alone or with family, with recommendations for home modificaations or equipment needs

Patient and family education to ensure safety with post treatment programs when discharged home

IN-HOUSE OCCUPATIONAL THERAPY

Bathing, dressing, grooming, toileting, eating, etc.

Help with instrumental activities such as managing shopping, cooking, medications, housework, laundry, driving, using public transportation, using the phone, and finances.

IN-HOUSE SPEECH THERAPY

Swallowing problems

LONG TERM CARE

 

SKILLED NURSING CARE

Available 24 hours a day, seven days a week

There are two types of nurses at Seasons: Registered Nurse (RN) and Licensed Practical Nurse (LPN). These nurses are responsible for the total care of the residents by initiating treatment plans and administering medicine. They prepare IVs, draw blood, and give injections. They will also give enemas, apply bandages and dressings, insert catheters, if needed, under the direction of the resident’s Medical Director.

Our CNAs (Certified Nursing Assistant) are responsible for the patient’s personal hygiene and day to day care. Their duties include, but are not limited to, getting the patient in and out of bed and moving them into a more comfortable position. 

CNAs also bathe residents, feed residents, walk the residents, help with personal hygiene, transport, and any other tasks that are designated by the nurse.
